Please explain your issue is in as much detail as possible.
A colony that is undergoing colonization but not actually colonized can somehow trigger 'Lost to Bureaucracy' even though it has no pops.

Steps to reproduce the issue.
1. Begin colonization of planet
2. Wait and get unlucky

Actually, the planet does have jobs occupied even though it's uncolonized. It has 3 pre-sapient jobs which I didn't know about. That could be causing the issue.
