I had an agent assigned to build a spy network on a primitive world, nothing else. I recently got the event stating she had been caught in a "salacious affair", with the options to revoke her diplomatic immunity or recall her for her own safety. Choosing the latter nets a -150 relations with the Iron Age primitives... despite them still having absolutely no idea aliens exist or even considering the possibility.
Steps to reproduce the issue.
1. Find primitives
2. Spy on primitives
3. Spy does something stupid which should, by all means, expose you
4. You are not exposed
