Thanks for the save. From a quick look at it, I think the original disappearance would be from the Odd Factory on the planet killing the only pop present. This would cause the planet to revert to the uncolonised state as you no longer had any pops present on it. You should have received an event about the Odd Factory being discovered. I suspect the second time, the Odd Factory killed the pop as colonisation completed, which is incredibly unlucky.

The issue should be solvable by either resettling a pop to the planet immediately upon colonisation or having the Colonization Fever tradition (which will ensure the newly settled colony has multiple pops present).
