I've checked and double checked for baronies; expelled Holy Orders, ECT.

Are there any other hidden requirements for this?

I finally found it!

The Duchy of Adrar which is de jure part of the Kingdom of Hausland and Empire of Mali and is not on ANY of the maps showing the required area was the final straw!

I wish I'd known and I'd have saved myself 3 times through the entire empire hunting baronies, as well as an extra war since I could have gotten it and Sous at the same time with an Invasion.

When I saw your comment about needing the Duchy of Adrar for this achievement, I thought "No way!". Then I checked the achievement code and saw how much land is part of the region of "world_africa_north". Didn't know that Trajan made it all the way down to the Canary Islands. ;) I'm glad to know this before I attempt this one for myself.

I'm still unsure what the "world_middle_east_jerusalem" region is exactly. Is that the entire "Near East" area that you need to create the Empire of Outremer?

Yeah-- all of the guides and wikis are out of date and none of them have the current exact list of duchies needed.

I know you don't need everything in Near East like you would for Outremer-- I have all of K. Jerusalem, K. Syria, K. Al Jazeera, and K. Iraq + Basra, (and of course K. Egypt) and Duchy Arabia but do NOT HAVE All of K. Arabia or any of K. Yemen.

(I did this achievement backwards- started in Flanders, got independence for Kingdom of Frisia for those achieves, eventually formed Holy Roman Empire, destroyed Byzantine in a Crusade, retook that area after it had fallen to Khazaria, made Carpathia + destroyed HRE, reformed Byzantine, THEN retook the provinces to from Rome + the borders).

You can find out which regions a county belongs to by clicking on that circular green button to right of the name

I feel so lucky for doing SPQR pre-india when reading stuff like this
I've did it before and after, I don't know how it's supposed to be more difficult as far as hunting baronies goes... I had to look for them in both. Second time around I knew what to look for.

The recent additions were a surprise to me though... I need to do an Orthodox SPQR now. :)

It wasn't the baronies that were the problem- it's really the out of date-ness of the guides which made it so I didn't know I needed things I needed.

After digging a little more, I think I figured out what is included in "world_middle_east_jersualem". It looks like there is a Jerusalem Region, which includes parts of de jure Syria. So, nowhere near as large as the "Near East" requirements for Outremer. See below:
