I can hope that the next DLC involves:

regents and regencies, which need a major upgrade, especially for player regents,
doctors, poisoners, and how these play into intrigue
about giving the players and AI rulers the means to punish those who insult them, say via seducing their wives, insulting their family, imprisoning or plotting against them,
similarly, a set of cultural opinions over what behaviour is acceptable in court, and how characters uphold or defy these traditions. So the king, say, could arrest someone for challenging him to a duel, but find that some of the nobility think it improper to have done so.

The general theme seems to be about a death, though I wouldn't be surprised if they tweak marriages a little and call the DLC "In Sickness and in Health".

A better looking map. It does not have to be SWMH-esque, but fixing the weird Danube in Hungary, the Dalmatian coast, the weird Rhine in Germany, the misplaced provinces in the Alps, the double provinces (eg Belgorod and Olbia, Pelusium and Farama, Tyrol and Innsbruck) and other mess hotspots on the map should be feasible? It was done for EUIV after all.

We are talking about features showed before official announcement. I remember what Paradox had shown before Conclave, so I am kinda sure death and sickness will be minor addition to next, peasant-themed DLC.

In the world of spying and intrigues there is still much opportunity for adding spice to the game.
(1) You could appoint trusted people, confided agents, whom you send to any person in the game for spying on him/her and revealing information useful to you. It is hinted at already by events, but I am thinking of a more direct way to produce evidence. Not connected to your focus on spying, as it is their talent which counts, not yours. And the use of this evidence could also be fine-tuned - for blackmailing cash, for forcing them to 'do you a favor' or demanding their altogether retirement from political involvement (so to say abstain and keep to a private life - but this requires very serious evidence like high-treason), or renounce a participation in intrigues and factions.
(2) You could order your courtiers to do things for you. Like seducing the wife of your opponent for causing him trouble in the family. Like hiring thugs over them for kidnapping his family members and asking ransom. Or simply having them sold to foreign slave traders. Or putting them by force into a monastery. That would bring you into some dependence of those courtiers, because they could threaten to tell that what they did for you once to others, as well, or change sides if you don't keep them happy. Depending on your espionage skills, you could handle a number of such personal agents. The element of fear is here also a thing that needs to be implemented. Use your personal agents for intimidating vassals, so they abstain from opposing you. Could work, or could make them join factions against you. This concept means adding a value to your court members you rather neglected in the past. Personal traits will become more important in that court, as it will become a pool of skilled agents for you. [edit:] To make that even more interesting, you may also spy on your spies just for testing them if they are honest with you or also work for somebody else - worst if they are double-agents for you and the fiend alike. Tricky grounds!
(3) You could use confidants also for positive things, like negotiating a deal between hostile houses. Settle conflicts in peaceful ways. This would earn you gratefulness of the parties, while you have to do salomonic decisions sometimes, finding a compromise in their clashing demands. Also could be used for proposing marriages and political pacts between third parties.
(4) You could propose a crusade against target region you specify in a message to the Pope. Maybe would need to be connected to a promise to send certain number of soldiers for that task and other benefits like bounty for the Church. Needless to say that region needs to be of non-Christian faith to be legitimate, preferably Muslim or pagan/heretic.
(5) I think there should be something like a 'King Arthur round table' where you assemble your most trusted vassals, courtiers and even foreigners for sort of an elite group within the realm based on personal friendship and shared ideals of chivalry. The members of that group will be most loyal to each other and you should be able to ask them for help in times of trouble. Troops, money, influence on troublesome vassals... [edit:] means also you have duties to them or they leave the round table/cabinet intimate club being disappointed. Belonging to this chosen few should mean high prestige for members. To get thrown out a substantial loss of prestige.
(6) Let's add some ways of calming down rebellious spirits in your realm by public events where you donate for projects like new cathedrals, hospitals, charity work or fine-tune taxes and contributions for single vassals. You may also build things for them benefitting them like royal roads through their territories or you grant rights to his towns to hold a fair which achieves an international fame and brings him advantages. Of course his gratefulness is expected in return.

If it's a religious-themed DLC we need a religious revamp. Early Christianity, Catholic/Orthodox split, Shia and Ibadi flavour and unique functions etc etc. Religious schools, monasteries, religious authorities becoming a new challenge to your power. And maybe Theocracy, at least in the Islamic sense.

Fix Flanders and Aquitaine!

I would like to see a fear mechanic. If my character did some atrocities, people around him should have a certain dose of fear towards him. As of now, it's far better to be a "good guy" and adhere to everyone's demands. If you are ruthless, you will most certainly have revolts. In the best case, people will give less gold.
Whenever people bring this up, I have one question: how do you balance this properly? If doing tyrannical stuff just makes your reign more stable, why not always do that? As of right now, you can already do just fine being tyrannical as hell if you know the game mechanics (especially post-Conclave. Council seats+NAPs is generally enough that a solid 50% of your realm simply cannot faction). Yeah, if you just do it wantonly, it's not worth it, but in plenty of cases the extra titles or gold you get can far outweigh the negatives. People are just so afraid of the "tyranny" modifier that they won't even try it.

And to complete the famous line, it is better to be feared than loved, so long as you are not hated. People always seem to forget that last bit.

One thing I'd definitely be in favor of is reevaluating cultures. What's the point of different ones? I always personally play and have my vassals as cultured of the lands they live in for the most part. I take the loyalty hit with them as I don't want to cause cultural genocide through assimilation (looks at the steppes). Only a few of them have unique features tied to them, like Altaic raiding, Byzantine mutilations and North Germanic / Norma raiding via the sea. Everyone else is basically SOL.

Additionally, the ability to actually execute people at times without tyranny. Rebels should be executable, or at least the leader of the rebellion without accruing tyranny. If someone seduces my wife, that should be grounds for execution or banishment without tyranny.

One last thing, rework Commanders. I think the position is fine, but I'd rather it either just give an opinion boost, or a +1 martial, ie. these are considered senior generals. Ideally, any male courtier (or female if allowed) should be able to become a commander of a unit with or without the title. Too few people die in combat as it is, this would help increase the odds I believe.

Since this dlc is all about death and stuff id like to see better attrition and more events for being on the road with the army. Ships sinking and more ways to die from plots would also be nice. Natural disasters could be really fun and play into the new unpopulated mechanics.

Bring on the crop failures, famine, earthquakes, sandstorms, snowstorms, regular storms, city fires, volcanoes, and maybe even tornado's (hey i was in one in Finland, they happen).

I would like to see Sahel kingdoms on the map... the connection of western and eastern Africa at the very bottom of the map. The Sahel kingdoms were really important these days especially because of slave trading which os omitted I do not know why...

And don't forget the part, that Machiavelli also said that the best would be the to loved and feared at the same time.

It should be:
High fear, low opinion: Many plots, few factions
Low fear, high opinion: Few plots, more factions (because you seem to be week)

To be stable you would need to have high opinion and high fear... but this should be nearly impossible ingame.

Whenever people bring this up, I have one question: how do you balance this properly? If doing tyrannical stuff just makes your reign more stable, why not always do that?
Fear would have different effect on different characters among your vassals. The proud will seek a way to fight you, the timid will chose to obey, for an example.
It is such an important factor in human character and again in politics, that it is strange 'fear' was omitted in game concept.
Tyrannic behavior will quench rebellious tendencies in a number of vassals who don't want to lose life, titles or belongings. We could connect that with disadvantages. Voluntary help and good-will contributions to your realm will be significantly reduced, as a feared tyrannt people try to keep safe distance to you and not expose themselves. Maybe less tax income unless you squeeze it out by force again. And this still makes peasant uprisings. Guys who have nothing to lose and thus are rather hard to keep under control compared to your rich vassals whose wealth and social position makes them vulnerable for you as their liege.

Another idea: I would suggest an elite retinue troop, which acts as your body guards. Effects: drastically reduced risk of getting assassinated, as these trusted men (well paid and chosen after loyalty and fighting qualities) will accompany the ruler all the time and may also protect close family members (wife and children). Treason from one of them should be extremely rare - still possible. Should be a privilege of kings and emperors, a reduced number maybe for dukes, couple of men only, but still could mean the difference of life or death to your character.

[Edit:] You could appoint members of your court to act as members of that body guard. A combination of character traits and high skills in military and espionage add to their value in this position, for countering assassination plots against you and your family.

[Edit2:] Includes a risk of your courtier-bodyguard to get harmed in the process of fencing off an assault on the targets he is protecting.

Currently in vanilla, only the Byzantine Emperor can imprison foreign spymasters trying to research tech in Constantinople. I propose that both the player and the AI can send their spymasters to any high tech province. The ruler of said high tech province can then get an event in which he or she can imprison foreign spymasters.

Fixed that for you. I've played Byzantine Emperor at least twice already......I never get that option.

But I see where you're coming from.

I'll go out on a limb and predict the new DLC will be called "Life and Death".

Maimed is getting expanded into various specific injuries... how about the same happens for Lunatic/Possessed?
