I just figured a player may be able to conquer Portugal but then after that there is really not much for Spain to do but watch?

A skilled player will definitely be able to get up to some interesting stuff with Spain, and in MP el cielo is the limit!

2. When the Spanish Civil War breaks out, a Spain player gets the option of whether to go with the Nationalists or the Republicans. If you the side you go with loses the war and is annexed, the game unfortunately ends.

I'm not sure if that's good from a game design point of view, but from a game like HOI it's definitely something I like.
Picking a side in any civil war will probably feel a lot like picking a side in real life.

well you could always save and reload as the other nation save if your playing ironman
