I have seen provinces controlled by rebels for a decade and never declare independence. I'm not sure if only historical countries can declare independence but I got the impression from the manual that any province in rebellion for a long time would do it.

There are a certain limit of available 'nations' with respective locations, that can pop up during the game. These are the ones that spring to mind right now.

Catalonia
Eire
Holstein
Hugenaughts
Mahrati
Netherlands
Norway
Provance
Royalists (Northern England)
Ukraine
USA

In the IGC there are a few more/new though.

Turkey, Russia, and Poland need to expand by war to compete

But they also recieve relativly much less BB points for their conquests, which balance it out (5-4:1 less BB per province taken on average, sometimes even more).

Have you actually taken time to look into it or are you just assuming ?

I've studied it quite extensivly and this has not been my impression so far, rather the opposite. I however agree on that you will get that impression by reading most of the posts about BB on this board. But generally the statements on BB has been highly exaggurated as the posters usually haven't botherd to look closer into the mechanics behind it.
Some have taken their time though and also contributed with accurate criticism of what might need to be changed to round of the ruff edges of the implemetation a bit. Some of it has been implented in the latest (1.08) patch and some have not and i suspect there will be additional tweaks in a 1.09 once people have had time to 'feel the latest changes out'.
