1. Builder bloodline- which holdings "count" for this- is it only temple-castle-city? Or do trade posts, hospitals, and or tribal holdings count?

2. How does the Asuna "prove themselves as mercenaries" bloodline effect work? Do I just have them assemble a mercenary company as normal?

Yes it's just the main three, and It's not a single bloodline, there are ones for completing the number for each type or some of each.

Doesn't it give you access to the nomads mechanic where you can have them assemble an independent merc company? I believe it's different from the standard version which you can do with any martial educated courtier.

1.- Answered
2.- Correct, you get the nomad option to "Send away as mercenary" for adult sons, or if a brother is heir, your adult brothers at court. Get this bloodline always.

Ok, so "send away" not "assemble" I'll look for the option for that; yeah- its and easy bloodline to get too since early game their sons count as lowborn so they have to matri-marry.

My Ivar Boneless start game I've gotten Ragnar (started with), Harold Fairhair, and Asuna combined, but all the Irish-English-Welsh lines and Rurik and Dyre are dead or dying.

I just got "Forge Bloodline" ambition, and I'm only at 4 foreign wars, but tribal so I can't decide what way to go for that one- I'm fairly young and did do wars and murder before age 12, but I'm a long way away from any of them and don't have the boats to be able to get off Britain realistically.

As for the historic ones, as mentioned elsewhere on related discussion threads, you can spend a hilarious amount of time at the game start wasting effort to keep historical bloodlines alive- I'll later this week share the new stories of building wonders, but let's just say, it started with me distributing Jewish Concubines to Dyre, Rurik, the Arpads and Harald Fairhair, to make sure they won't die out, and I am right now pondering if I should not just subjugate some tiny county in the west end to have diplomatic range to the British bloodlines before they die out.
Gonna have an easier time though this run- Dyre for once in MY lifetime did not start as demon worshipper, but with the wolf warriors, so there's hopes he wont just bring all his kids to the forest this run!

As for the forged bloodline, this is a trickier question indeed. As mentioned above, you can in theory forge new ones later in the game, and even combine them, but this is a generation-spanning project, and using a bloodline on your subsequent rulers is also a more than just viable option.
To ease up your decision, I'll simply refer to the complete list of bloodlines on the wiki rather than to give a recommendation- You'll see yourself what would be most useful for your playthrough.
If you want this humble builder's opinion, though, naturally Murder-Builder is the way to go :D

As you'll see however, there are a lot that have significant advantages, so it's really taste and pick.

Yeah, I thought I had some of the British ones saved- got them married in right even, but then some of them didn't have kids and now they are too small, too old, and too many of them are Monks since I booted them all off their land.

Dyre and Rurik were both in the 8-12 range so I thought they were fine, but then poof gone in one generation while I was distracted conquering England.

i also have a Charlemagne start where I was trying to save them all, which if that's the goal it seems like Catholic or Byzantine is better even though the Norse bloodlines are "better".

Yeah, before I've always forged that " 15 foreign wars" one, but this time by the time I actually hit 5K prestige I had all but a tiny, abused Mercia conquered on the British isles, and like I said- not enough boats to even move 3K troops, so I'm kind of stuck--

Seems my options are murder, builder, or going for peace.
