What really stands out is the rate at which I have to replace commanders, have six as a kingdom and I'm replacing one every year at least not counting combat deaths, which is far more quickly than I ever recall. Only one game post-patch so might be confirmation bias but I suspect not.

You guys do realize that you can request that your physician treat you every 5 years, right? I've had Syphilis for the past 40 years, and every 5 years I get treated by my physician, and probably ~80% of the time I get +2 health (and +2 stats) for the next five years. Even the "bad" result is only -1 health for 1 year. Just stick with the common treatment and you'll be fine.

Come on people. For the first time you don't stand with the same ruler for 50 years and be sure the the same will happen with the next. Also you understand that cancer is one of the most causes of deaths along heart attack even today.

Also in three of my games i had many rulers who lived for many years. Just not almost all of them like it was before patch. Actually before patch if i had a ruler die from illness i was really happy indeed cause it awakened from my slumber:p.

As for the commanders... One commander each year? Of course! They are the commanders. Thay die in battle most time and thats how it should be! ( i had a great king die in battle...but that was amazing. )

At last CK II is a game...with challenge, and not just something that you play until you learn the rules and mechanics and then conquer the world!

I love it! Loaded up a Byzantine save at around 1270's. At this point I had rolled over my neighbors and was just mopping up.

Then comes the Black Death...man, I'm bleeding money due to my large retinue, my vassals are dropping left and right, the ones that are left are in rebellion (I decided to test the new torture events). Then my king dies, leaving a 2 year old daughter in charge and I get a jihad called against me.

Fun times!

300 year game....and now my dinasty was wiped out hahahah i love this dlc my king was castrated, my wife dead, all my 9 kids dead...my heir became my first daughter who i married to a distant lord to save her from death. I was forced to play an old save and now im building that hospital and begin the preparations...now i know how to deal with it...right? Hahahha madness.

Cancer is common today because people live longer and most other diseases have been cured to some extent, it would have been less common rather than more in the middle ages.

Ok, I think that I'll modify my earlier comments and say that things should at least be tweaked somewhat with regards to non-epidemics. Here is a list of my rulers, how old they were and what they died from:

Count Raimond of Melgueil - Died Age 54, had: Great Pox, Lunatic, Severely Injured
Duke Peire of Toulouse - Died Age 49, had: Incapable
Duke Raimond IV of Toulouse - Died age 26, of the Plague
Duke Ermengau - Died age 67, had Great Pox, Lunatic
Duke Guigues - Died 61, Incapable (for like 6 years!!!!)
Queen Ermessentz of Aragon - Lunatic, Incapable at 53

Game over, because my son had married his daughter Ermessentz in a traditional marriage, and his only son died of the plague. Was literally 1 year away from switching to Elective........

So literally half of my rulers had the Lunatic trait, and so did half of them slip into incapable before they died. That seems high... 1/3rd also had the Great Pox.

I LOVE the Black Death (although I had it happen at 1100, and then not again until my game ended at 1200), and I like the epidemics, but the normal diseases seem a bit much. Just my $0.02.

edit: Played with "delayed dynamic" for the plague, and it only showed up once at 1100.

This is insane. All my sons dead again but i managed to give titles to some obscure members of my dinasty so i still are playing with my 300yo game...

But i lost muh empire. At least i managed to make a reformed germanic europe..

Tried playing as Nantes just because I wanted to form a small kingdom and make it as prosperous as possible. Made it 5 years before dying of Stress, Cancer and Infirm.

My character has had "chest pains" for about 25 years, but haven't been able to request treatment the last 20 years (it always says "can't have been treated recently").

I think the death rate is good, if anything i was a little disappointed at the start of my new campaign because I didn't feel like enough characters were dying when I scanned through the map :/ But then the black plague hit and it was great. I'm using the default epidemic setting

My first game with TRD was as the King of Sweden in the last bookmark. The family is small (only 4 members, 1 male, matrilineal off) and he died in battle shortly after. It was game over within 30 years but I love that!

And not including the black plague, the death rates for characters seem better than they were before. So one question to confirm: with The Reapers Due, are illnesses more common and deadly now? I wasn't 100% sure of that before. I appreciate it because I'd rather not see christian dynasties extend to have 40+ members and grow even further.

Remember, the new custom settings are pretty helpful because you can reduce how frequent outbreaks are :)

As for the commanders... One commander each year? Of course! They are the commanders. Thay die in battle most time and thats how it should be! ( i had a great king die in battle...but that was amazing. )

As I said in my post,that's not counting combat deaths which are of course to be expected. It's just something that prompts you so you notice the higher mortality.

In my first post-Reaper game, I've had about seven or eight children. Not a single one has lived to see the age of 30. Those that haven't hit 30 yet though, they're all lunatics or depressed.
The Reaper has come to take its due, with compounding interest.

Pdox should just get an overall statistic of character's deaths and adjust it accordingly. More weather related and infection deaths, less cancer and etc.

Also, tone child death the fuck down. I know there were a lot of infancy deaths in the middle-ages which a google search tells me was probably around one third, assuming the scholars and whatnot are right. On ck2 it feels like It's over a half, and It's overall pretty ridiculous that noble children on a medieval game die more than on studies including piss-poor peasants.

I mean, at least give us the option to tone it down!

I think the death rate is good, if anything i was a little disappointed at the start of my new campaign because I didn't feel like enough characters were dying when I scanned through the map :/ But then the black plague hit and it was great. I'm using the default epidemic setting

My first game with TRD was as the King of Sweden in the last bookmark. The family is small (only 4 members, 1 male, matrilineal off) and he died in battle shortly after. It was game over within 30 years but I love that!

And not including the black plague, the death rates for characters seem better than they were before. So one question to confirm: with The Reapers Due, are illnesses more common and deadly now? I wasn't 100% sure of that before. I appreciate it because I'd rather not see christian dynasties extend to have 40+ members and grow even further.

Remember, the new custom settings are pretty helpful because you can reduce how frequent outbreaks are :)

This makes things more interesting. In my current game as Count Robert Capet of Bourbon (Charlemagne start) my first wife got cancer and died a year before our only child was diagnosed with it at the ripe old age of 4. After treatment, she lived until she married her lifelon suitor at the age of 17. Shortly before her death, my character was diagnosed with cancer. Being brave he lived about 15 years off of experimental treatments and the life essence of his cat and dog until the last cure involved cutting off his leg and apparently the cancer. After surviving the severe wound and subsequent infection, now Duke Robert (I presume he got the title because his battle powers and badassery demanded it) is pushing 70 and is the oldest noble in the realm. Oh and his first born son died shortly after becoming an adult due to the flu and his last surviving child/son is attractive.

Overall, it's been great fun. I just wish I can request or order my court physician to keep treat my family.

Come on people. For the first time you don't stand with the same ruler for 50 years and be sure the the same will happen with the next. Also you understand that cancer is one of the most causes of deaths along heart attack even today.

Also in three of my games i had many rulers who lived for many years. Just not almost all of them like it was before patch. Actually before patch if i had a ruler die from illness i was really happy indeed cause it awakened from my slumber:p.

At last CK II is a game...with challenge, and not just something that you play until you learn the rules and mechanics and then conquer the world!

...It isn't a "challenge" if the game is just constantly throwing new illnesses at you
Being thrown into a regency because your fifth severe illness (again, NOT epidemic, that would be understandable) in as many years finally rolled the "bedridden illness" card and killed you is just tedious and not even accurate
Forcing a restart because of something that happens at random on a large scale can't possibly be the new disease feature Working As Intended. That sort of thing would normally be considered a serious flaw in a game

The thing about commanders, I don't think the issue is they die in battle, they certainly didn't die that often on Thursday. Also, battles tended to be pretty one sided before things like gunpowder were invented so they shouldn't even die that often in battle unless the player really sucks at choosing good targets (although, sometimes bad luck happens and that just happens)

At the moment I can't get a headache without it turning out to be super-Cancer, and that's insane. The severe illnesses are as common as the "ill" trait was yet more deadly
Ideally what I'd personally prefer were symptoms being as common (or even more common), but have them lead to full blown illness less
That way you still have the potential for death and the physician is really important, but there's less being totally at the mercy of the RNG
