One thing I have found is that a good court physician have a huge impact on survivability, so most of my close relatives have survived due to having a good physician, but my vassasls seem to struggle quite a bit, and I have also had a fair share of vassal child rulers (it might have more to do with starting in Charlemagne start where few have had the time to breed decently sized families though)

Anyway, I wonder if the amount of people in court (and whether they have diseases) impact how often people become sick? I have a tendency to invite loads of random well-educated, well-bred, filty rich, dwarfs and other funny people to my court.

Health is definately more important and having lots of children is not a luxury. Also martial education and selecting struggle focus to try and get brawny is kinda worth it too, especially for your second/third sons who can either end up being great commanders or your backup plan in case your territory gets hit by epidemic and your dynasty is at risk of dying out.

Hunting focus is also stronger than ever because it essentially gives you +2 health. I'm just wondering, does having a good hospital in your capital help a lot? I know it gives the county disease/epidemic resistance, but I assume it also helps the characters in your court.

p.s. Watch out with getting on the council of your liege. You'll switch courts, get a different court physician and might put you in the middle of an epidemic.

I have to say, I really enjoy the death rate, it makes primogeniture WAY more interesting and who you get as your heir a bit more of a tossup. PLUS, it makes having a ruler who ruled for 25 years feel like a hell of an accomplishment.

The last few games I had were very tedious because I had to keep updating/re-assigning my council missions due to members constantly dying.
I am honestly afraid of what will happen if I try to play a duchy larger than Barcelona, and have to deal with constantly doling out titles.
The UI is not really conducive to such large degree (and now required?) micro management.
After about 150 years, there were more child rulers (or rulers with regents) than adult ones.
I hope its just a bug, or the AI's inability to deal with the new sickness mechanics, and not how the game was intended to be played now...

I don't mind dying occasionally from a random serious illness, my issue is that I'm getting illnesses literally once a year to the point that the physician really shouldn't be doing anything other than just following me around
I've restarted 5-6 times, always have the same issue, it's impossible to make any future planning (the constant deaths by illness combined with the AI's over-willingness to overthrow established dynasties means that there's no possible way to know who the next ruler of anywhere could possibly be)
I'm constantly stuck with regencies because living PAST 25 (never mind ruling for 25 years) is an accomplishment

I have hospitals, I'm playing on default settings and I have an amazing physician. I'm scared to see what happens when an actual epidemic hits if this is what it's like just playing the game normally


EDIT: The usual argument for this would be that the average life expectancy was low, but that's more to do with the sheer number of infant deaths than any actual information on how long the average person lived

One problem I've noticed is that the AI isn't building hospitals. Not sure if its related to AI reasoning or budget restrictions though.

On the other hand some people being lucky doesn't actually mean that there isn't an issue, that emperor probably had Amazing Health and, well, he did get cancer. Just because they don't kill people immideatly doesn't mean that the diseases are not too common, which is the main issue.

As Caspoi said, I've survived cancer in game, I've survived dysentry in game, I've survived pneumonia in game, I've survived flu in game
The issue isn't that diseases happen and sometimes kill me, but when they happen all in the space of a few years eventually the RNG eventually gets a bad roll and I end up bedridden in my 20s
I'm not looking for them to make diseases less deadly, they should be as deadly as they are right now (ie sometimes you live, sometimes you die, mainly based on luck)
I'm looking for them to be less common (maybe 3-4 severe illnesses over a lifetime outside of epidemics)

I, for one, welcome our new child-overlords.

They are much easier to assassinate

But to be more serious.
My Characters all tend to go hunting when they start to feel sick.
It sounds weird but it seems to help them overcome that aweful diarrhea.

Edit: Or they spend more time with their family... or the family spends more time with them on the toilet... you know, whatever works.

I've noticed the majority of sicknesses, being prevalant as they are, are offset my the treatment health bonus. So, yes, sickness does seem a little out of control at first glance, but you're, for the most part, relatively safe from them as long as you have a competent physician and stick to well-tested treatment. A slight problem with this system is that the AI is less likely to agree to beneficial marriages because you're sick.

Don't know if it's just because I am playing a nomad but I haven't really noticed much of a difference in death rates. Though I may have been lucky my first character lived to 74 despite being a lunatic with the great pox. I also had a wife with a headache, my court physician managed to kill her trying to cure her so I burned him at the stake :D

The death rate rose by quite a bit with the new DLC (though not necessarily in an annyoing way).

Until now, my by far most preferred succession law was agnatic ultimogeniture. It was as reliable as primogeniture, provided your wife is not (much) younger than you, with the added benefit that your new ruler is usually in his 20s and not in his 40s/50s. I almost never had regencies (i.e. not a single one i my last few campaigns).

Now, death comes more randomly with illnesses, botched surgery etc. I´m 150 years into my new campaign and had already 2 regencies with ultimogeniture, so i guess i´ll have to change my habit now..

Just turn the diseases down in the options. That is what they are there for!

I hate shattered retreat. So I turn it off. You do same for diseases. Just turn them to low or off completely.

Problem solved.

If I'm not mistaken, the rules only affect epidemics, which are currently fine, not the regular diseases like cancer or leprosy.
