Nope. You can't customise realms. You can customise your character.

With the customizer dlc you can change the name of your realm but I guess we'll get to rename our titles, crowns and such.

Why? A new start date provides fresh challenges. Its a new scenario to play in rather than keeping it with the same old one. The 867 start is a very different start to the 1066 original start.

Charlemagne wasn't exactly on anybody's list; a historical character driven expansion wasn't on anybody's list. I thought people wanted more diplomacy, intrigue, RP elements, more story events for THEIR OWN CHARACTERS, deeper feudal mechanics, more on the Black Death and the Church, etc?

Usually only stuff that doesn't effect the core-gameplay for everyone no matter if you have the DLC or not end up being put in the DLC, with LoR as the exception. Taking this into account i think it is pretty safe to assume that the main thing you'll be paying for is the new bookmark
