I've reformed Slavic Paganism and I've choosen Polygamy and Autocephaly

After some time (few decades) Slavic Church was without Head and only regional Patriarchs and Matriarchs were available.

Is this by design cause I choose Autocephaly?

Yeah that's the idea of Autocephaly. Each region will have their own head. Maybe by independent Kingdom, so Poland will have it's own head, Bohemia its own as well. If you control both there would just be the one for both.

I think the head is also just whoever you choose as your court chaplain.

Could be wrong about this, just what I remember from playing as Orthodox in lands that aren't Byzantium.

There is supposed to be a set of patriarchs. The Orthodox Religious Head title can be destroyed under some - rare, buggy - circumstances. But in that case, there are a decision to recreate it. I don't know the requirements but it´s certainly tied to Byzantium or Constantinople in some way...
