I don't know, if it's a bug, an old bug or a new "feature":
Erik VII of Denmark doesn't hold the PUs with Norway and Sweden due negative prestige. Now, Norway and Sweden were free countries on their own. Then, I saw the first strange stuff: At the death of one of the monarchs, Erik VII succeeds to the throne, who is still the king of Denmark. It will be no PU. So, now the king of Sweden died with no heir and Erik VII succeeds to the throne with no PU. Now, he is ruling two kingdoms separately.
I looked around and saw the same thing between Naples and Aragon. Maybe it has to do with PUs from the start of the game?
It's WAD?
