What do i have to put into those fields?

Make three entries:

Enabled   Description   Inbound port     Type    Private IP Address    Private port
 check     HoI2/1        47400-47700       TCP      your comp ip       same as inbound
 check     HoI2/2          2300-2400       TCP      your comp ip       same as inbound
 check     HoI2/3        28800-29000       TCP      your comp ip       same as inbound

I believe this is exactly what you need. Let us know how it worked.

doesnt work, when a friend wants to connect it says "Cannot find at (my IP)" blah

firewall is off, antivirus is off. do i have to open ports?

Does your friend have proper setup? I.e. are you sure your setup desn't work, did you try with somebody else?

I got a D-Link DIR-635 and I haven't got it working. I've tried OP's ports and those in #301. I once got it working for me to create a server and my bud to join it but not reverse. It stopped working some time after.
