So, as it happens, there are actually a couple of characters in CK2 who are descended from notable figures of classical Roman history. Three come to my mind especially, those being the Salvius and Pius houses of Ragusa, and the Galbaio house of Venice. Two of these are in 769, so I guess they're slightly oriented towards earlier dates - and that's only fitting really. My proposal here gives a few options, with a simpler one being something like a European equivalent of the Parthian bloodlines, and the more complex one relating to the actual people they're descended from.
So, the basic gist of these bloodlines is that, as is par for the course in the Middle Ages, everyone wants to be a Roman. With these three dynasties, there are actual people with verifiable lineage to Roman emperors themselves! Now, what sort of things should a generic Roman bloodline have? Well, here's my vague idea of how it could be:
1) Prestige boost, of course. Being descended from Roman emperors is just the best, so possibly a significant prestige boost.
2) Opinion bonuses. The obvious choice is boni for Christians and Hellenists, possibly for Greeks, Dalmatians, and Italians too - representing the close-to-Roman, pseudo-Roman, and Byzantospheric cultures, the Romans and the sub-Romans, compared to the more distant cultures like French and Spanish ones which undergo more distant development and have their own regional things going on.
3) Perhaps some more 'tangible' benefit. Perhaps converting counties to your culture faster? The Stoics have the "Civilize" ability, and this was something that the Romans and their descendants struggled with constantly. The Dalmatians were in a constant war to keep their own identity, language, and political status separate and above those of the Slavs living in the hinterland, while the Venetians (and arguably, the Italians broadly) tried their own hand at colonizing across the Mediterranean. They might also have some access to a softer, more limited form of the Imperial Reconquest CB - or alternatively could just have an easier time fabricating claims to represent the legitimacy of Rome to nearly the whole continent.
4) They could be able to do Roman Renaissance or Delve Into Classics regardless of their culture, as long as all other conditions are met. Alternatively, Delve Into Classics could be a "free" action without any requirements, framed as these characters looking to their Roman heritage to study appropriate skills or mythology. The AI would, of course, stick to skill boosts for the most part. Alternatively alternatively, this bloodline could be a condition to replace insanity and scholarship as requirements.
These are all just suggestions, and I apologize for the relatively low effort of this post. The idea just struck me, and I wanted to write this all down before I lost it. As a minor final note: The Palaemonaitis dynasty in Lithuania could potentially have the option to forge a Roman bloodline if they convert to Christianity, representing the historical attempt to connect Lithuania to the Romans as a way of proving some noble origin instead of being the barbarians christianized by German blood that the Poles and Teutons wrote.
