How do I restore a disabled icon in top of the screen? I accidently disabled the icon which indicated that you could create one or more inactive titles/duchies, and I can't find how to put it back. Restarting a new game doesn't work either.

to the right - outliner(info box) - configure categories - disabled alerts- click the one now displayed in the box - done.

Disabled alerts can be re enabled by the using the settings of the centre right panel and showing disabled alerts. Then simply click the alert that was disabled.
