Yesterday, I tried to request an invasion CB for the first time ask the pope for his blessings to invade the theocracy of Norway... The tooltip said he would approve, I lost my piety, yet a few days later the pope sent me a letter back saying: "No." Is this how it is supposed to work?

It happened to me once as well. It should be due to some changes in the days between the request and the reply that affects the relations. What should fix this is that piety is lost only on request's approval.

Or maybe not, just to give that additional element of risk and randomness.

The "yes" corresponds to the situation at the time of your request. It is possible that circumstances have changed (eg the Pope had lifted the excommunication on the King-Bishop of Norway during the interim period between your request and the reply) which results in the eventual "no" you received.

It is considered a bug by many.

Grand him a County or Duchy and make him rebel.
Now he hates you but is independent... oh and make sure he isn't conquered by anyone. Have fun

I think the AI may notice that you asked for an invasion and send a gift to the Pope in an effort to equalise relations. This is why the "Request" often fails when the tooltip says it should work. Not that what anybody said is incorrect, but there is a very specific change that often occurs between the request and the reply.
