I think the picture is self explanatory but I will explain. I own multiple provinces with many cultures. One of these cultures is Novgorodian. All my provinces are also Orthodox. I have multiple Novgorodian Orthodox provinces. The problem is some Novgorodian Orthodoxy provinces have Religious Society province modifier and other Novgorodian Orthodoxy provinces have Heretic Culture province modifier. I also do not have have Novgorodian as an accepted culture just Ruthenian as my primary with lithuanian and byelorussian as accepted.

Were any of these lands owned by vassals you integrated? Also this is more bug report so repost there please

There seems to be a general bug of province modifiers from estate privileges not being properly removed, like the Mughals still having the Dhimmi conversion penalty despite no longer having a Dhimmi estate.

There seems to be a general bug of province modifiers from estate privileges not being properly removed, like the Mughals still having the Dhimmi conversion penalty despite no longer having a Dhimmi estate.

There's also a recurring bug where estate modifiers are not always placed in new provinces: I had the "no conversion" dhimmi right which worked on all current heathen provinces but not ones I gained after giving the right. There was a similar issue with the Tropical City Planning right.
