I have restored Roman Empire and have many viceroys below me. The empire and all kingdoms/viceroyalties have the Controlled Realm Inheritance law active (which disallows a title to pass to another realm). But counties keep switching between viceroys (which is messing up my internal borders and create too powerful vassals)

For example:
I granted the Viceroyalty of Sicily to Lazzaro, who is duke of Sicily and has some counties in this kingdom. His great cousin Aldo (they have the same grandfather) is the Viceroy of Italy, is duke of Milan and has some counties in his kingdom.
Ten years later, Aldo dies. And (probably because no other heirs were available), Lazzaro inherited Aldo's titles. The viceroyalty of Italy came back to me (as it should), but the duchy of Milan and all Aldo's counties are now Lazzaro's and appear as the Viceroyalty of Sicily.

Since the kingdom of Italy has Regulated Inheritance, and since this realm's law is the one active (I destroyed the Empire of Italia and the kingdom has not yet dejure-drifted under the Roman Empire), why could this happen ?
I know the titles did not leave my realm but it did leave the kingdom of Italy...
