So in my current Dax game, my ruler got the infirm trait at the ripe old age of 43...and 1 year later, slipped into a coma and went into regency.

At first his son, the primary heir, took over as regent. This was fine because I was grooming him to rule anyway and had very healthy stewardship and dipomacy ratings.

After 2 years of regency, my wife suddenly announced that she would make a better regent and took over. I married a character to shore up my character's weaknesses. so she had high learning and intrigue...but poor stewardship...suddenly I have to shrink my holdings or suffer a diplo penalty.

How did she take over as regent? My son is still the primary heir, and he didn't die or have anything odd happen to him... so why the sudden change?

There is an event series where characters can challenge the current regent to replace him or her. They get the chance to challenge this in court, or fight a duel. Likely your wife did the court route and won.
