You can get black plague + dialogues for the DLC even if you don't have the dlc.

The Black Death as such is (should) not (be) a bug, aka bubonic plague had been in the game before the DLC and is still, as far as i know and as it should be.

It shouldn't be (have not tested in game) as devastating as for the lack of hospitals.
That the option for the hospital is there might indeed be a bug.

The black plague starts when you build that hospital with the dialogue with a head of a religious order.
Which is weird, because no hospital is built anywhere, and everyone does (it's the DEADLY dlc plague)
