I don't no why but this has always bothered me. That Titular Kingdom Titles have the same Rank and Authority as Realm titles. Real Titles to me are unifying Titles. Because they included all the Land. Titular's do not included the land and have No DJ. To me there needs to be an in-between Duke and King to solve this. Principalities were common in those days. So you would have Duchy's-Principality(Titular)-Kingdom(Realm). Once the Realm Title or unifying Title is created. It is the Top of the food chain. Principality would be 2 duchy Titles(Non-Titular). (Doesn't matter how Much Land) If you got subjugated by a King rank (an actually King) Then you just go to his realm and you get to keep both of you duchies. Example Whales is not really kingdom realm it is apart of England Whales in fact is Principality(We don't say the King of Whales we say the Prince of Whales). I just get tied of having to chase all the Titular titles in my realm and them Destroying them. Because they keep splitting my Realm up. And it shouldn't be that way. Or some Vassal gets all the land to create one of these Titular King Titles through succession and Creates it and goes Independent. And theirs nothing I can do . If I attack him or take his land away I'm tyrant.

I'm not sure there's a problem here. You don't get to rule a kingdom without being either a tyrant at home or a conqueror abroad. Usually you need to be both.

At the same time, everyone beneath you is angling to build up a power base large enough to conquer you. Titular titles represent those individuals capable enough to to make themselves the de facto sovereign of their lands. They are, deliberately, an equal of a du jour king.

Either keep your vassals weak and scattered enough (including brining them into your court with the express purpose of marrying them off to someone nonthreatening, or killing their spouse and children so there's never an inheritance) that they never amass a grand duchy in the first place, or let them grow knowing you'll have to destroy them later using either tyranny or intrigue.
