After racking up a few hundred hours now I am starting to notice that nearly all realms are far too stable and peaceful, even when rebellions break out unless the nation has been brought to its knees the rebels are barely a worry.
I would like to see more of the competition and rivalry of nobles houses, the church, merchants and to an increasing extent at the late game the peasents. I think the factors that often caused issues within realms were slights and insults that the various parties felt and acted upon.
We have issues like noble killed, help out nobles, various church v noble and noble v merchant events but this never seems to affect your realm for any length of time. I feel if we had to manage the internal politics of the realm more and there was more danger in favouring a certain family or class that this would mean that realms were more likely to have issues.
Also I find it strange that armies are entirely loyal, especially in far flung corners of the world. I would like to see armies flip, like if a general decides to usurp control over Cuba and takes his army with him. It would just create more danger in the world.
