I'm playing a game as the Papal States in the 1.13 beta so I'm of course biased in this matter but I think the rank of the papacy should be set to emperor. Historically the Popes set themselves up above the kings of Europe and conflict with the Emperors over precedence was an enduring issue, though one more so during the CK2 period.

While the Roman patriarch has always had a powerful ecclesiastic authority, his (and maybe "her" a time or two) temporal authority as represented by the Papal States is more like that of a kingdom based on current mechanics. The Pope already gets bonuses and has his own government type, I'm not sure why more is required.

The Papal states often called themselves the kingdom of god, not unlike the descision but I can't think of them claiming the title of empire of god. The papal states are powerful enough as it is, the empire rank might make them even more so, able to conquer quicker as they are less restricted by autonomy.
