With the newest patch+expansion, my wife suddenly got a disease.
She also got a small green modifier symbol - first it was called something like "good treatment" and then later it changed to "mild treatment".

Was is this modifier? What effects it, and why does it change?
I have an idea that it is caused by my physician, but I dont get any info on it sadly...

When she fell ill, your physician treated her. The reason her treatment level lowered is because he treated her again, not as good this time, and the decent treatment took precedence.

That's pretty much it.

Ok, makes sense, thanks... But I dont get any information on this in the game? Or a decision how he should treat her?

Not sure, but when a close family member gets sick, you have the option to care about it or ignore it (Death takes us all).

I imagine it represents him doing his usual thing and him not doing a thing.

You only get a say in your own treatment. When close family is treated you get an event that lets you essentially either say, "That's nice," or, "I don't want to see these events anymore." If the physician botches the treatment of a close family member, you can imprison/kill them depending on traits, or lower the opinion of either the physician or the patient and raise the opinion of the opposite.
