Does anyone know why when occasionally you marry a family member or courtier off to a King, the female character face does not update with a crown after the marriage? Is this a well-known bug, or is there some reason for it? In the particular case I noticed it, I married off a half-sister to a dynasty member King when he came of age who owes fealty to me as Emperor... they were also bethrothed before.

Well, there are numerous graphical bugs in the game involving people not refreshing their graphics. I've noticed this with monarchs who go lead an army, don a helmet, then never, or only much later, take it off even after they no longer lead the army.
The coronations take place a few weeks or even months after they become Queen so that's why the time delay. You don't get to wear the crown as soon as you become Queen. You have to move to the location, get married and then have a coronation.
