Were you previously using the the Family Relations (FRE) mini-mod? Did you stop using FRE at some point and perhaps only just now notice the lack of an opinion modifier for that in vanilla and HIP/PB?Were parent-child relation bonuses removed? Sadly, without tweaking the approximately two game-global knobs that would totally flatten all interesting activity in the game if turned further down (in terms of AI aggressiveness), Paradox gives modders almost zero ability to mod actual AI decision-making [or strategy], unlike some other games. There is little we can do about the described problem other than to encourage you to accept that sometimes there are bad rulers, and generally speaking, the AI is always a poor ruler compared to a moderately sane human being (or their traits, political options, internal realm dynamic, and natural AI tendencies happen to fit what a human ruler would do-- luck, in other words).Could you tone down AI aggressiveness by a huge degree in general and when they are not ready to wage a war?
What are the fabricate claim requirements? I recall there was once a tooltip ala war declarations (unless I'm misremembering) but right now there isn't and I can't send my chancellor after anyone.
Also, with a previous ruler I met all the holy war requirements in the tooltip, but still couldn't declare holy war. Was it a religion thing (I'm playing as the Khazars, and if you took out holy wars for Jews I can see why) or some other issue? My current ruler doesn't have the traits, but one who does may come up again.
(For now, unable to declare war in either way, having pressed all my de jure claims, and with a religion which makes inviting claimants to land and press claims for difficult to say the least, I'm faced with a bit of a waiting game.)

It's probably because I deleted the continents file, so it messes things up trying to check for a continent that doesn't exist. It's a result of bad vanilla mechanics. Not really much we can do about it I'm afraid.Is this intended/vanilla behavior?
Waging a holy war against a king. One of the contested counties declares a "War against the Tyranny of X" against their liege, my target enemy in the holy war. I win the holy war, but do not gain the rebelling county, which immediately surrenders to its old liege and continues to exist as part of the old kingdom.
Now I can't declare war for it for 5+ years, and to make matters worse I had that county completely sieged out.
Rumite Decadence Revolt has been going on for 30 years and the attacker is in an Unknown Location. I kill him off through the console and he's replaced with a new war leader, still in an Unknown Location.

Most of the titles you list no longer exist since they got removed in 3.5.0. I guess they might still exist in VIET Immersion.Could have something to do with what I was about to report...
In a non-SMWH (with Viet) install using the current build, I have lots of missing provinces. They're in landed_titles files, but not in province_table.csv or in localization files. Some have rulers set in the start file, some don't.
Here's a list of what currently is lacking in localization files, but which exists in landed_titles files. Some, like d_frisia, seem to simply lack localization. The vast majority (including, I believe, all the counties) are phantom provinces that simply screw stuff up):
It's really frustrating finding out that you can't create a duchy because you don't control counties that don't exist.
While I'm tempted to do surgery on the landed_titles files and my current save file to cut out the nonexistent provinces, in the short term I at least threw together a quick localization file for the missing titles:
At least this way when I search for a county that starts with "c", I wont ever have to see non-localized names in game. *sigh*
