Prepared Invasion failed. Why?

When you prepare an invasion, you have 2 years of "preparations" during which you get additional troops and boats. Within this period you must declare the war against your target using Prepared Invasion casus belli. It does not start automatically. I remember I failed my first PI like that.
There could be other reasons, but it's hard to tell without knowing what happened.

You must also not become hostile before declaring the war, so don't raid them to soften them up beforehand.
