I thought of making Byzantine maritime republics, to rival the Italian ones.
I was thinking of Rhodes, Athens, Smyrna and Costantinople itself.

The Byzantine Empire would become a semi-feudal crowned republic of sorts.

Seriously, Byzantium as a whole would actually work better as a »republic« than as a feudal empire — especially since the ruling dynasty never, ever changes, which is fairly implausible. On the other hand, a Byzantine Republic with the Imperial Reconquest CB (trololol) and the Maritime Republic CB, as well as a huge trade network all across the Eastern Mediterranean, strikes me as silly and probably imbalanced.

Accually if the empire is inherited by a mayor or a doge. It should be possible.. I remember a game where the Doge of Venice became the Grand Doge of the Holy Roman Empire.

The Isle of Man would make a great republic to dominate trade in the british isles. Investing money in 4 major countries and influencing things would be nice, although there is the huge risk of Scotland attempting to annex. I will mod in the Republic of Man for fun ;P.
Republic of Rome also, ofc, but it isnt viable because of the papacy. Great for a mod tho.

Ive also seem a grand doge of the HRE based on Pisa (who's rise to the throne triggered my very own 30 years war btw), so Im sure a bizantine grand doge is possible, but given the restrictions theyre putting on making republics playable, you cant really say for sure unless you consider modding and save game editing.

The first Dev diary said that the Feudal and republican Holdings remain seperate within the one Family for purposes of inheritance; However as a republican leader can hold both City and Castle holdings I think those would go to one heir while any Feudal inheritance would go to another who is then removed frim the Republican line of succession.
While Patricians have a bourgeois background and are not proper members of the nobility, they are still important players in European medieval politics. Thus, they can secure alliances through marriage just like feudal lords. However, they cannot marry matrilineally and any marriage proposals made to the nobility must be accompanied by an appropriate bride price, which can be pretty steep. Moreover, children born to such couples can not inherit both the Patrician holdings and a feudal domain. What tends to happen is that one child becomes a lord and another takes over as Patrician (although Patricians are limited to Agnatic Seniority succession, so it's more likely an older relative becomes the next Patrician.)

However, if a single title such as the HRE or ERE becomes elective I am unsure what exactly would occur.

My default choice for a republic would be a coastal region where the local dynasty has died out and there are no serious claimants. That means leaving the people mostly alone under my mostly nominal supervision, as long as I get taxes and levies.

Also, for obvious reasons, Republic of Ireland would be a blast.

Cherson, as an isolated and ignored theme of the Byzantine empire, located on the scenic Crimean coast and in a prime position to dominate Black Sea trade. In real life it was something close to a Merchant Republic (or Free City) during the time of the game, enjoying an autonomous status within the Empire, with the Archontes (Magnates) electing one of their number as Proteuon (leader). So it is an interesting position, protected by the Byzantines with a lot of room to expand, surrounded by pagans and heretics and able to dominate the Black Sea, plus it is grounded in historical reality. That will probably be my first game as a created republic.

I was going to make Cherson too, since Genoa holds one of its counties later in the game with the chance to create the whole Republic and then break way while the played character is not the Serene Doge of Genoa itself. I'd personally attack the Cumans or the Golden Horde and attempt to form the Most Serene Republic of Taurica. From there I'd probably use trade across the eastern Mediterranean and black sea to back up an invasion of de jure Alania and Cumania. Although I would have Italian culture and Catholic religion from the start, I'd probably end up going native by the end and being Mongol, Alanian, Greek, Russian, Cuman or Pecheneg.
