Patrician cannot take decision look for court physician

Just started playing a patrician in Venice, no holdings only the patrician mansion. And I do not have the decision to look for a court physician.
Yes I can spam look for holy man but that does not guaranty I find someone with over 15 learning.

Just now my second wife died of a pneumonia since she could not get any treatment. (My first wife also died, but she was a cheating harlot and was pregnant of some bastard child (it saved me from having to do an assassination), so don't really care about that one). If I get sick now I also can not get any treatment.

Is this wad? because if so it is really annoying.

Sounds like an issue with barony-level gameplay. The solution is to conquer more.

Seems like an oversight. The search for a doctor event doesn't guarantee you'll get someone with 15 or more learning though. I've rejected several and gone with priests instead.
