You'll need to change grant_limit to greater than 0, at which point it's grantable, and change opinion_effect to something positive. You also may want to add
So it's not grantable to people who shouldn't be able to get it - women (sorry ladies), children, muslims/byzantines (who would have a different title that does something similar, although you'd have to write it yourself). Throw in some 'trait =' virtues for it too, so you don't have a slothful lunatic as your paragon of chivalry.
A random courtier is one thing but I don't think, realistically speaking, that a duke or count would get the 'court dwarf' designation from his king back in the middle ages.
