And you are sure you have completely logged out and then back in, and scrolled though all pages of your owned items. I looked in the back end and see it in your account.
And you are absolutely sure you reviewed each of the 96 entries in your owned items list.

Great news gang, the Crusader Kings II Limited Collectors Edition is now sorted and everyone who bought it should have their icon.
Still the 500k club, CKII Holy Knight, and EU4 Pre-order icons missing.

If you link your Steam account to your Paradox account and then navigate to your "Owned Items" tab on the forum, you now have a "Sync Owned Items" button. Click it and every Paradox title you own will automatically be registered to your account! How awesome is that!

That actually added my Pride of Nations icon, despite it being impossible to register Pride of Nations keys normally.
Sadly didn't do the same for Rise of Prussia though, nor for several games that never gave me a key at all:
- Achtung Panzer
- King Arthur II
- Penumbra
- Elven Legacy
Synced again after getting The Cossacks; didn't add it to my icon list. Bug? (Redeemed it manually after nothing happened upon syncing. Still not showing up for whatever reason)
The syncing system glitched; I had it as my only medal for a bit.
Resyncing a few times got rid of it and restored my regular icons though.

We no longer own the rights to Achtung: Panzer! and as such we have removed it from the Owned Items list. This means you will all see one icon less from now on.

I've actually reinstated the Owned Item, however, the problem is that users that got the icon from linking their Steam account will see it go missing since Steam looks at our games in the Steam database. Users who bought it way back and got a separate forum key for it will still have the icon though.

Yes since the current way it works is that it looks at your Steam library. The icons are meant to represent Paradox games and technically, when we no longer own the rights to it, it's not a Paradox game.
I do see both sides of the coin though and understand that some users buy games just to get the icons. However as I have explained several times in the past generating keys isn't feasible and we can't start handing out keys for products we no longer own the rights to. It's not ideal but at the moment it's the only explanation I can provide.

Not a bad idea at all, I'll make a note of it!
"Soon" ><. I actually don't have a better answer and it's sadly not as simple as copy pasting a list of names from the old forum and giving them item X. Some of the icons had quite large user groups and that means crediting several thousands (or hundreds of thousands!) with an icon is a fairly large undertaking. Trust me though, I've not forgotten about it, not in a million years, but I can't tell you any specific time frame.
