I saw Papal States conquered and annexed many times, but it's ridiculous! I know that sometimes Papal states were defeated, occupied in this period but no one dared to annex them and expell Pope from Rome! It became possible only in time of nationalism, revolutionary ideology etc.
Firstly all rulers who attack Papal States should be automatically excomunicated ( if they are catholic), secondly after annexation there should be event for conqueror to give back Rome province, if rejected it should give very high opinion penalty with all catholic rulers, there also should be special casus belli for all catholic countries to declare war to liberate Papal States.
Finally decision to unite Italy shouldn't require Rome! Historically Italy formed without Rome, and they had capital in Florence. There were also catholic who wanted united Italy but oppossed conquering Rome.

EU3 had a cutoff for the Holy War CB for 1650 (near as makes no difference Peace of Westphalia) which felt like a good cutoff for the decreasing importance of religion in European politics. Possibly before that point have a negative Triggered Modifier for anyone Catholic other than the Papal States holding Rome, plus a CB for all (other) Catholics to restore the Pope should he not hold Rome?

I'd agree with this completely. There should be definite penalties for a catholic conquering Rome and annexing it like it were just anywhere else.

If memory serves, CK2 needed Rome to form Italy? (Perhaps not though) and EU3 gave a bonus for it, rather than a penalty.

I think its right that the papacy isnt destroyed and just re-emerges elsewhere, but there should be huge religious turmoil and diplomacy penalties for annexing Rome.
