This is the campaign you're playing here? There are a number of things in the campaign that aren't in the sandbox mode. For shotguns you need armed guards.

Yes, you should probably change it to an armed guard for this, to make it accurate

Old thread but can I advise against this change? The campaign is a tutorial and armed guards dont get introduced until Chapter 3 so it is not an issue seeing a normal guard with a shotgun yet.
The fact a normal guard gets one can be explained by the staff rushing to offer Palermo whatever protection when he gets sent to the infirmary.
