Twice in one game this has happened to me.
I was the King of Ireland, Scotland, Wales, and Brittany. There was a revolt going on for years in one of my vassals Dutchies. The leader of the revolt, and the claimant were also my vassals. Eventually the revolt won and usurped the dutchy. But suddenly the new Duke though Scottish is a vassal of Poland and I can't try take it back because we are allies.
I reloaded and tried to stop it but I can't interfere with my vassals wars. However this time after the title was usurped he stayed my vassal. No idea what was different.

Then again after I formed my empire, a dispute was fought over a dutchy in Wales between two vassals and suddenly the HRE, my allies, controlled the region.

What is happening here? How come my land can be taken without me having a chance to defend myself? I can't press claims on nonindependent titles without fighting their liege? How do I prevent it happening to me?

I would appreciate any help these forums can give. Thank you.

Not sure regarding the first one, but the second sounds like someone in the HRE might have inherited the title in question (instead of the war resolving).

Did they control the majority of the land under the duchy? It is fair game for them to usurp it if they control more than you.

Nope. No territories in the dutchy, or any land nearby.
The first time happened along the English/Scottish border when Poland was given it. (They have no land outside what they would normally have though)
Second time is there in Powys.

If the revolter inherited a title in another realm, then won the revolt, that might cause this? That seems a little unlikely to happen twice though.

He has no titles from the other realm. The only change on the character is his opinion of me (because all the factors related to vassals are gone) and he has a new liege.
Right before the revolt ends, I am his liege.

I can't see the image in your previous post, but did the person leading the revolt definitely get the title? Is it possible that he formed a faction for some claimant to be installed who had land in a foreign realm?

Maybe try posting the before and after saves?

I'll try reposting the image and the saves when I get home.

The first time it wasn't the leader of the revolt who got the title but it was still one of my vassals with no claims or titles in the other Kingdom.
This time it was the leader taking the title. There seems to be no mention of another Kingdom until they usurp the title then suddenly have a new liege.
