If you are allied to both the attacker and defender in a war, you should have the option to declare neutrality in a war. This prevents you from participating in either side or being called in, while still maintaining your alliance with both sides. You only get a small opinion penalty with both and a small prestige loss. The defender of the faith can choose to declare neutrality as well without giving up the title, but they suffer a bigger penalty.
This is something that the AI could also do, and depends on various opinion factors.
Subjects, guarantees, and imperial membership take precedence over this and the overlord, protector, or emperor cannot choose to proclaim neutrality in this case.
