Thousands? Are you kidding me? Even the USA would probably fall if its ten biggest cities got nuked. In 1945 Truman planned to drop several more bombs on the Japanese cities and when you don't have nukes, it is only a matter of time before you surrender. Also, you just need ONE bomber (or one rocket) with an a-bomb that manages to get past the AA-fire. It's not THAT hard.

That doesn't, of course, make them economical weapons. They cost a lot, they must be carefully monitored and they destroy everything on their path, so it's no wonder they are not widely used. However, as I've said, they can win the war for you. The problem with HOI2 is the fact that neither the player nor the AI have any reservations from gameplay perspective, because the game usually ends in 1940s and there is no point in playing beyond that. Therefore, unlike in Europa Universalis III (which has a very long timeline), the future doesn't matter much.

I don't know how DH team wants to handle nuclear warfare, but it's obvious that they are poorly handled in HOI2 ATM and if the DH team wants to tackle the Cold War period, then we need an improved and more complex system. In HOI2 terms, Vietnam would got nuked in 1960s and the war would be over in a month or two.
