Earlier today I was continuing my game and something happened that got me thinking. My scarred warrior Duke was leading his levies in battle when he was wounded again. It made me think how nice it would be if there were levels of scarring to increase on the scars from 20+ years earlier. But then time passed and my mind got wandering and I've fleshed that out and added more to it! So here's my thoughts.

Lightly scarred
DESC: This character has sustained wounds that have left him/her with minor scarring.
EFFECT: Monthly Prestige +0.05 (monthly)

Scarred
DESC: Old wounds have left this character visibly scarred.
EFFECT: Monthly Prestige +0.10 (monthly)
NOTES: The same as the current scarred trait.

Heavily Scarred
DESC: This character has sustained many wounds over the years that have left them with a series of scars.
EFFECT: Monthly Prestige +0.20 (monthly)
Health: -0.25

Badly Scarred
DESC: Many wounds have left this character with a countless amount of scars.
EFFECT: Monthly Prestige +0.25 (monthly)
Health: -0.50
Attraction Opinion: -5
NOTES: Would require a higher scarring effect for the portrait.

Any time a character that is wounded has the wound heal there is a chance that it will be one of the new scarred levels or one of the new maimed levels. If he's already scarred then there is higher chance to get a higher level of scarring.

Also, an idea for a new congenital trait, Mute.

I don't think many people survived some of these wounds, especially if done by an enemy on the battlefield. Losing a leg for instance. Not to mention this would need some kind of way to represent infection if you want to get this specific. It wasn't until WWI that soldiers survived major injuries like losing their leg, leading to a prosthetic revolution of sorts. During the later period in EUIV it's more plausible, but how many Kings are there in history who survived their arm or leg being chopped off? Pretty sure the enemy combatant would follow that with a fatal blow, yet considering the time period they already did. Bloodletting especially increased the instance of soldiers dying, and Kings would get the best bloodletting of course. The system in place is ok enough, but some things could be implemented. A paraplegic would be an interesting trait to have. They would be unable to lead armies, but not unable to rule or reproduce like the blinded and castrated.

Paralyzed would definitely be an interesting addition and there are plenty of instances of this as a result of horse falls, etc. But frankly I think that's already pretty well modeled, if more abstractly than OP is looking for, with Infirm and Incapable.

On the subject of toes:

I've broken many of mine at different points over my life. Even those injuries, without loss of them entirely, has left me with issues. There is a significant loss of 'transfer' of downward force into forward force which makes my gait uneven. The pads under the metatarsals (just behind the toes themselves) will swell and 'burn' after long periods of standing, etc. Even novice students of most forms of combat will tell you that applying upper body strength into force very much involves use of the hips. How much 'drive' you can get through your hips depends a lot on footwork and a solid vertical base. The toes actually do a *lot* more work than most people realize. The average human is putting 16 PSI of force into the ground at rest. Now add heavy armor. Now swing a sword with enough force to overpower an opponent's block (which, since every action has an equal and opposite reaction, eventually means you are pushing against the ground).

To be fair, the descriptions for these are about losing the "use of" the limb in question, not the loss of the limb outright. I get the impression it is describing an injury that might mean you could still pen a letter or gesticulate with that arm, but attempting serious combat with it would be comically ineffective.
