The icon could be the same as the icon for the Aztec religion from the CK2->EU4 converter. I have two possible versions of the idea - vanilla and more ambitious.
Vanilla version:
Base bonuses:
Tolerance of the True Faith +1
Manpower recovery speed +10%
Local missionary strength +2% (like other pagans, it makes them more susceptible to conversion)
Heretic rebels: Jaguar cult (in-line with other pagan heretics being named after animals)
Unique CB: Garland War
You cannot take land, but winning the war gives you extra prestige, legitimacy and +1 stability, winning a Garland war against a rival gives extra +10 power projection (decaying at 1 per year).
The religious decision religious_sacrifices would now be linked to the Mesoamerican religion, rather than to the Aztec, Maya, Zapotec, Inca and Chimu tags.
A more ambitious version:
New mechanics: Divine favour – this value represents how strongly people believe you to have support of the gods
Goes from 0 to 100%, has a base yearly decay of 5%, positive stability lowers the decay by 1% per point of stability, negative stability increases the decay
The bonuses at 100% are:
+0.2 goods produced nationally
+20% army morale
-20% army recruitment time
The decision religious_sacrifices would be removed, instead you'd have a button to organize religious sacrifices which would give you 10% divine favour, at the cost of increasing global revolt risk by 1% for 2 years – you cannot use the button while the effect is active.
Unique CB: Garland War
You cannot take land, but winning the war gives you extra prestige and 30% divine favour, winning a Garland war against a rival gives extra +10 power projection (decaying at 1 per year).
Additional effect of temples. Having temples in all of your provinces gives +1% divine favour/year. Having them in less than 100% provinces gives a proportionally smaller bonus (similar to how inflation reduction from Counting house works).

Trigger: On battle won
After the battle your men gather all the captives. As custom dictates, some of them are to be sent to the temple, where their blood will nourish the Gods, while the rest will become slaves. Perhaps one day, they too will have the honour of ensuring that the Sun travels across the sky.
Option A: A worthy capture.

Lack of captives
Trigger: On battle won
While the battle was won it seems that the Gods will not receive their fair share this time. Too many men have died and there are barely any captives that can be send to the temples.
Option A: A disturbing news.

Tlacaxipehualiztli
Trigger: around March
The great Xipe Totec flayed himself to feed our ancestors and so, as the night and day become equal we must choose those that will be sacrificed to him and flayed as he was at the great festival of Tlacaxipehualiztli. Their skins will be worn by the priests for twenty days and when the celebration is over they will be preserved in the depths of the temple.
Option A: Choose a common slave.
+5 divine favour
Option B: Take the most noble prisoner.
+10 divine favour, lose 15% yearly income
Toxcatl
Trigger: around May
The time has come once again to celebrate the Festival of Toxcatl. A young man, chosen during the last year’s festival to be the ixiptlatli – the impersonator of the God Tezcatlipoca will parade through the city and he will be sacrificed atop the temple. His skull will be placed on the tzompantli skull rack and his flesh will be distributed among the high nobles.
Option A: I will retire to my palace, as the custom dictates.
+5 divine favour, -25 administrative points
Option B: I will participate in the festival.
-5 divine favour
Atamalqualiztli
Trigger: around October or November, does not have the “Festival of the Water Pancakes” modifier
It may be hard to believe, but it has already been eight years since the last Atamalqualiztli and so it is time for another fast. For a week, the only food eaten will be unseasoned tamales soaked in water. Our other food will rest and be revitalised, and any who do not stick to the fast will be punished with leprosy.
Option A: Good thing it’s just one week.
+5 divine favour, - 20 of each monarch point type, gets “Festival of the Water Pancakes” modifier giving +1 yearly divine favour for 8 years
Option B: A full week with nothing but wet tamales? That’s just cruel.
