I'm trying to create a new division. I have enough food, arms, money and manpower but the 'OK' button is grayed out. What else would keep me from building a new division?

government policy if you are playing revolutions - anti-military and pacifist parties can not build during peacetime.

Hmm, I did just have an election. I am playing Revolutions as Brazil. The current party is Partido Conservador. Their policy is Jingoism. How can I tell in-game if that's the problem?

That is wrong.

You can always build new divisions or ships even if you have a party that is anti-military or pacifist.

You cannot promote soldiers or officers as anti-military during peace time.

You cannot promote soldiers or officers as pacifist at all.

It might be because the culture of the division you are trying to build can't support the division.

You can tell how many divisions each nationality can support by hovering your mouse over your manpower icon.

I think that's it. I have four division and when I hover over the manpower icon it says 4/3. What exactly does that mean and how would I increase the number of divisions allowed?

Promote more population units of the desired culture into soldiers.

4/3 AFAIK means that you have 4 units of that culture while you only have soldiers to support 3 of them. Try converting more POPs of that culture to soldiers.
