When I formed Netherlands my monarchy government changed to a republic and I lost the ability to form royal marriages. I also got crappy leaders (1,1,4) for many years.

At some point my government switched back to a monarchy. Excellent!

I am wondering if this is always the case or was it a random event?

It's difficult to tell you what event it was without giving us some more information.

Netherlands gets a unique event with a mean time to happen of eight years. I presume this is the event that you had.

It gives you four options:
Dutch Republic
Administrative Republic
Administrative Monarchy
Do nothing

You chose the second option if you were getting 1/1/4 etc. That was a bad choice. You want Dutch Republic until at least the Age of Absolutism. Then you might want to become a monarchy again.

But too late. The event only happens once.

On that event I think I would have chosen Dutch Republic in order to experience that nations historical events. It was a long time ago so I can't be certain what I chose.

I was unable to change government out of a republic. I am not aware I did anything to cause my government to revert to monarchy. I just saw that I started getting random leaders and heirs again.

You had those crappy rulers because you did not reelect. Playing as Republics is a bit tricky, especially at start. But that's definitely what you went for.

There are usually popups and/or alerts when things start changing in your realm. To my knowledge you don't just flip from Republic to Monarchy with no warning or explanation. Perhaps you were re-electing your rulers and became a Republican Dictatorship when your Republican Tradition went below the threshold. And when the dictator dies if your Republican Tradition is still low then you will become a Monarchy again.

Dutch republic is a bad govt form right now since it rolls 0-5 for stats. That's right, you will pretty much never get a 6 stat dutch ruler with republic
