One of the big immersion breakers for me in CK2 and 3 alike is the apparent lack of any sense of naming conventions for higher ranking rulers. Let the AI Karlings or Capets rule France for a century or 2, for example. Reading back over the title history, you're not as likely to find multiple Henris, Charleses, Louises, etc. as a bunch of kings with the most random-ass names like Centule, Manasses, and Evrard. Certainly for many European cultures there should be a clearer sense of royal/imperial naming conventions which lower-ranking rulers also follow to a lesser degree. Also, cultures like Bulgarian just need more historical names in their namelists. You can randomly generate baby names for quite a while as a Bulgarian before you encounter one that was historically used by their royalty, especially for women.
Even Greek needs more actual Byzantine era names rather than whimsical classical era names like Aristarchus.
