So I played a few 100 years with RD, all the rules at Default except for Sunset, Great Plague and Mongol invasion - all random.

If I started a new game I think I would set the epidemics to more rare, I find its "oh here we go again, epidemic coming my way". It feels like a familiar grind after a while and its very easy to get craven from it (Lunatic actually helps in that event).

Hospitals are super expensive. I can't identify with the people who say your ruler not living very long, if you always go into seclusion and have a good physician you can live quite long even without taking + health foci.

Overall the death rate is higher, but it most seems to be affecting the AI who is not as smart about disease prevention. You do need to have more extra family members around to make sure your dynasty does not die out. On the other hand the game is in a way easier since its easier to just let yourself or that imbecile heir die from a disease.

The christian physician events need to be toned down a bit, it causes too early religious conversions. The same with the immortality event chain, it should be super rare. Missing limbs should also be a bit rarer.
Some people talked about how elective monarchy spread has been fixed. At least not in my game. Several major realms turned elective and I was called into the 6th Francian civil war for Elective monarchy.

One thing which is immersion break as heck is that for certain mild illness you can get an excellent treatment which makes you better of on net than without illness. The treatment should never do more than remove the negative effects of illness.

Overall illness should maybe be a bit rarer, and cancer should not be very common before old age, its just immersion breaking.

Cancer should be linked to age. Like, characters under 30 years old with cancer should be extremely rare, rare up to 40, and past 70 it would reach it maximal occurrency... which shouldn't be all that high, either, but much higher than for younger characters. in a more or les lineal fashion.

I have not found anything really bothering me with this new DLC...

I started as the Doux of Bavaria(now King of Bavaria) and found only minor problems. The biggest issue was me as King not being able to push a de jure claim from the kingdom against a theocracy of Apulia. It was not possibly before one of my vassals held the Douchy of Carinthia and i had to push his claim, then.

Dieseases were no big problem for my ruler - with great court physicians and a well built hospital in Oberbayern i only lost one infant heir to the consumption and my 50something ruler has had the great pox for 15 years...

I see almost avery realm going to Primogenitur after ~100 years of play. But the HRE was extremely stable in my game with the first civil war just after really 100 years....

Seems a bit odd, but i can live with that. The higher Death rate has the biggest impact on claimants, i think. Most duchies in the HRE only have 3 to 5 claimants so it is not that easy to get expand.
And once i was invited to carousing by my dear friend, the doux of Swabia for maybe 30 times in a row..... pushed both our diplomacy by roughly 5 or 6 points :).
(i played without defensive pacts or AI seduction and AI intrigue)

Just a supplementary comment. I'm not sure if I experienced more epidemics due to being in Holland.
More prosperous areas get more disease right?
So many outbreak seemed to be centered around the english channel.

According to a Paradox dev, being tribal means you practically don't have to worry about the disease outbreaks. He made the point that he's currently playing as a city razing nation, which ironically helps reduce the spread of disease, despite his savage ways.

This is a blessing because many here were complaining over how tribals could begin to even afford hospitals. The answer is they don't have to. Or so I've heard.
