There were a few prestigious Imperial titles in history, which are fun to roleplay:
Play a Balkan nation, get Constantinople and declare youself the Basileos. Or unify Persia and be the Shehinshah. Or take Cairo, Bagdad, Damascus, Mecca and be the Caliph. Not to mention the Celestial Emperor or the Khakhan.
These are easy to implement (I added them to EUIII myself), and they are extra nice when they come together with some small bonuses. They also supply a goal to play for, which is kind of lacking in EU series, especially for nations outside Western Europe.
The Western Imperial claim (despite being a complete fraud-
) has its own mechanics and vast bonuses in the EU games. These other titles are somehow ignored. Don't add new mechanics by all means, but titles with conditions-small rewards, for instance similar to the 30-province imperial administration decision, but with more flavour.
