There was a mod (Holy See or something) where they added new province and i would like to know how did they do that?

As a ghost province, yes. But they didn't change the map. Others mod has done this as well; Cold war mod, Modern Day Scenario, HIP, etc.

yup, it did exist, using the "ghost" provinces, kind of like the German factions in Kaiserreich.
