When meeting Hieronymus to give him the elucidation stones after doing Blade Grave old walls (a breach between worlds quest) there is a bug in the second dialog choice that goes like this:
1- I found the stones of elucidation.
2- *missing 01_cv_hieronymus148*.
3- I must take my leave.
Version is V 1.2.0.0133
