I'm gonna have to assume it has to do with Milan forming thr ambrosian republic, but that's not supposed to happen while they're in a PU
Correct, it's a same-day decision. I've seen this before.

Immediately after their leader dies without an heir or queen they fall into a regency and check the decision to turn into a Republic.
However, if they're in the tier where they'd get PU'd they also fall under a PU, which they can't since they're a Republic.
If it gets contested their new "overlord" has to fight a succession war without getting the PU.

Incredibly rare, but it happens just like OP described it.
Dunno if it's new (don't think so) and can't remember if I reported it before, but it's a bug.

EDIT: Could be fixed by delaying the decision by one day. Only that specific decision though. Other decisions should stay instant for various reasons.

Just got this bug aswell. Personal Union under France, but I got the event so I chose the republic. So now the french king is my Captain & Defender.

Edit: Realised the top post said electing another ended PU. Also, personally I think the republic should have priority over PU. But I haven't really thought much about it since, well, it just happend.

Well, the story behind it is what makes it for me.
The milanese duke dies with his french and castillian family members hawking around his bed. The nobles have already decided that upon death, Milan will be a new noble republic. The french king however, has some legitimacy claiming the throne of Milan and the nobles manage to trick the french king that he indeed shall be on the throne... for his entire period. The king is satesfied. As the castillian king hears of this, he declares war since he also has a claim to the throne. The french and castillian armies clash around the pyrenees and Paris is occupied by the burgundians whom the french where fighting before. Four years later, the nobles gather for the first real election where they convene and elect the dimplomatic candidate which terminates the french claim on the throne. But the french have much already forgotten the slight since the french king died shortly after and his son obviously had no interest in the milanese throne. So the war against Castille ended with some war reperations. Milan now stands a bastion of influence and humanism at the heart of europe.

... but yeah, the bug should be fixed. Harmless in some ways yes. It resets enemies which isn't too bad I guess (shame I lost my PP though). It makes a strange succession war fire since AI never declines to jump in. It is also a way to buff the royal family since reelecting the regent holding the PU will increase the monarch points. So while it is mayhaps not something that is easily orchestrated, I can see how, say, Austria gets the PU over the player with Milan and if both are players they decide "Lets win together" and just keep reelecting the now guaranteed 6-6-6 Habsburg family to be Captain & Defender.

It's harmless if you are Milan, you can end the PU when you want just electing another king, but if you are the overlord is not funny .

Sadly the only way I found to not get this was to restart from the _backup like 15 times until they didn't take the ambrosia republic. After that my game as Albania has been one of my best games ever. I'm at 1600 being the #2 great power
