I blew 1k gold into getting Defender of the Faith in my war against the Ottoman Empire and a few years later with almost all Ottoman territories conquered I noticed that I no longer had the title. There was no notification!!! That needs to be changed.

The patch notes say I can lose the title by losing a war.

How do I lose a war? I was winning every battle and signed no treaties except a peace treaty that forced Vassalization on a one-province country.

Are you sure the Turks were the alliance leader? Perhaps you made peace with a smaller country, which was in fact the alliance leader (therefore, you made peace with the Turks).

I've noticed in the chronology of monarchs that the game doesn't consider vassalization to be a victory...

If you demand terms then is should not count as a loss. Demanding someone to become a vassal is not losing it just happens to be what you demanded.

If you settle for white peace or offer tribute then that should count as a loss. In my opinion anything else should be considered a bug and not WAD.

If it is WAD then it is stupid. Unless I am missing something.

If the option of Vassalization is accepted by an enemy under the make peace choice of 'Make Demands', then that should count as a Victory for game conditions such as Defender of the Faith and the chronology of monarchs.

I mean, considering what forced vassalization gives the winner in the latest version of the game (defeated nation becomes vassal = 1/2 tax revenue goes to new liege, automatically will accept Join Alliance offer by liege, etc), it's clearly not a draw or defeat for the nation making the demands.

Could someone please post a Bug Report regarding this if the system as it stands considers a purely vassalization offer for demands not a victory?

Well I just did a test: loaded as Austria, cheated for some cash, became defender, dowed salzburg and force vassalised them. My title stayed. So vassalisation doesn't seem to be the cause.

In my opinion, the following system should be used to decide the victor of a war, if it si not already:

If you propose and they accept a peace where you issue demands, or accept a peace in which they offer concessions, you have won.

If you propose and they accept a peace where you offer concessions, or accept a peace in which they issue demands, you have lost.

If they declare war on you and you sign a white peace, you have won.

If you declare war on them and you sign a white peace, you have lost.
