Playing as Germany, if I conquer Poland, then liberate it, which provinces do I get to keep?

All released puppets always get their 'core' territory, plus any extras you may currently hold, EXCEPT core national territory of the releasing power.
Therefore, if Germany releases Poland, Poland will usually get the core 'Congress Poland' area plus anything to the east, usually to Minsk. They won't get any territory to the west as Germany has cores (usually) on this land.
