So I did it folks. I achieved immortality in my ironman Roman Empire S.P.Q.R. Achievement run (which I am now wishing I had done way back when it was easier). I am now one of the 0.04% of the folks that have this achievement. It is actually super convenient for where I'm at, as I have over 25 vassal kings and now I don't have to worry about stability problems with succession anymore. All Hail God Empress Isabella! She continues to bear fruit of the womb at 55+.
So I wanted to discuss how the event chain works and what I've deduced from it so that you may also achieve immortality--or at least avoid an untimely demise. Feel free to contribute what you have discerned as well. This chain is highly costly, and it represents likely the largest gamble chain the in game thus far as you have a very high chance of failing. This can result in you being maimed, mangled, depressed, killed, or unwillingly reincarnated (i.e. killed).
1. When the event fires how the event chain will progress will depend on the stats of your advisors. If you have amazing advisors you will have an opportunity to have them investigate immortality for you. If you have multiple options you will want to pick the advisor whose stat YOU have highest. So if you have high diplomacy and can pick the chancellor, go with him.
2. As the events go on you will be apprenticed to a courtier of some non-Christian variety (if Christian, I haven't done this as a non-Christian). The courtier that comes corresponds to the following advisor:
Chancellor = Beautiful Norse Girl
Marshal = African Warrior Woman
Spymaster = Muslim Crone (often hunchbacked and/or ugly)
Chaplain = Buddhist/Hindu Precocious Child (implied conscientious reincarnate)
Steward = ?
What the steward brings I do not know yet, as that is the one path I haven't taken (if it exists).
3. There will be two events spaced a few months to a year apart which involve some form of training and/or proving your merit before actually starting the final chain. If you fail at any point in these the quest will cancel and you will likely get away with minimal consequences, but death and maiming are always something of a possibility. These events will give you the chance to obtain traits. For example, you can get Brawny (success) or Frail (failure) through the Norse Girl quest.
*Take note that in my experience most of the options to throw money at the quest typically fail. I think that paying is just a pure RNG if it ever succeeds at all, and I think in some cases the choice equals failure always. The other options will always succeed as a function/probability of your attribute level for that exercise (i.e. pickpocketing for the intrigue path).
4. If you succeed in the first trials you will initiate the final event path which will have 2-3 events in close succession.
Diplomacy = Seeking out a sacred apple of Iðunn in Scandinavia.
Martial & Learning = Drinking a magical potion.
Intrigue = Synthesizing the Philosopher's Stone
Stewardship = ?
The final events success, which will determine whether you gain immortality or (almost always) die hinges upon your stat for that path. I am pretty much convinced that if your stat is lower than 25 you will likely fail. If you succeed, you will gain immortality. This will halt your aging and make you impervious to disease. I am not sure of all the details, but I acquired immortality in my mid-twenties, and have continued with a young portrait and birthing children far into my fifties.
Keep in mind you can still die randomly from events that fire, such as the crying for help while drunk event.
So basically my recommendation is, if you are attached to your ruler or them dying in that 2 year time frame would induce catastrophic consequences, then it is probably best if you just abort the mission. If they do not have a stat over 25 it is also likely not worth pursuing unless you want unfortunate things to befall your ruler.
